Electrical Stimulation

Whenever you are recovering from an injury, stroke, fibromyalgia pain, or any soft tissue conditions, one of the most beneficial treatments recommended is a physical therapy procedure known as Electrical Stimulation (ESTIM). The painless procedure of ESTIM accelerates recovery and relieves pain from several symptoms, and is widely used by physical therapists in every rehabilitation program.

ESTIM stimulates electric pulses into the body to mimic the action of signals coming from your nervous system. Muscles and nerves are the main targets of these electric pulses. These mild electrical pulses help train your muscles to respond to natural signals of the body.

Types of Electrical Stimulation

  • TENS (Transcutaneous Electrical Nerve Stimulation): Electrodes are placed on the skin targeting the sources of pain. When the electrodes stimulate the pulses to nerve fibers, they block or reduce pain signals reaching the brain. TENS therapy is useful in treating both chronic and acute pain.
  • EMS (Electrical Muscle Stimulation): Similar to TENS, but uses rhythmic contractions to aid in building muscle strength. EMS uses a slightly stronger current than TENS to contract muscles.
  • ESTR (Electrical Stimulation of Tissue Repair): Used to relieve pain by reducing swelling and increasing blood circulation.
  • IFC (Interferential Current): Pulses beneficial in reducing pain.
  • NMES (Neuromuscular Electrical Stimulation): Useful in increasing strength and restoring muscle functions.
  • FES (Functional Electrical Stimulation): Aims at providing long-term stimulation to preserve muscle and motor skills.
  • SCS ( Spinal Cord Stimulation): Relieves spine-related pain and discomfort.

How Does ESTIM Work?

ESTIM uses small electrodes — sticky pads placed on the skin of the affected area with wires attached to the ESTIM device. A stream of electric pulses reaches the muscles causing them to contract. The pulses aimed at the nervous system stop the pain signals from reaching the spinal cord and brain. Natural pain-relieving chemicals called endorphins are also stimulated by these electric pulses. The process typically takes around 5 to 15 minutes.

How it's performed

Self-adhesive electrode pads are placed on the skin over or near the target area and connected to a small stimulator. A controlled stream of low-voltage electrical pulses is delivered, and the parameters differ by mode. TENS targets sensory nerves to modulate pain signaling and prompt endorphin release; EMS and NMES use stronger currents to produce muscle contractions for strengthening and re-education; interferential current uses crossing medium-frequency currents for deeper, more comfortable penetration. Intensity is set to a strong but comfortable level — a tingling or tapping sensation, never painful. A session usually runs five to fifteen minutes and is often combined with manual therapy and exercise.

How to prepare

No preparation is required, and there is no fasting. The skin should be clean and intact where the pads are placed. Tell the clinician about any pacemaker or implanted device, pregnancy, epilepsy, or areas of numbness before treatment.

What to expect after

Response is reviewed over the course of visits, with pain and functional measures guiding whether to continue and whether a home TENS unit is worthwhile. As an adjunct, its continued use depends on whether the overall plan is helping; it is not relied on as a sole treatment. Patients whose pain does not respond are reassessed and, where indicated, referred for further evaluation.

Outcome

Patients commonly report reduced pain and muscle relaxation during and shortly after treatment, and muscle-stimulation modes can help maintain or rebuild strength during rehabilitation. The evidence is moderate: a large recent meta-analysis found TENS lowers pain intensity versus placebo, though guideline bodies have been cautious because many older trials were small. It is a low-risk, drug-free adjunct that works best within an active, exercise-based plan.

Can I use a TENS unit at home?

Who should not have electrical stimulation?

It is not used over a pacemaker or implanted defibrillator, the front of the neck, the head, or a pregnant uterus, and it is avoided over cancer, infection, clots, or numb areas. We screen for these first.
